Some people are intelligent, self possessed adults by the time they’re 20, other people are immature shitbags well into their old age. Age is a poor metric for evaluating compatibility.
There’s nothing about surgery that’s addictive. You’re addicted to the validation you feel, which is a sign of deeper unresolved issues relating to self-worth. Surgery can never resolve those underlying issues, but surgeons will keep taking money to tweak just a little more. You’re never going to solve a psychological problem with surgery alone; you need to explore the roots of your feelings of inadequacy, identify why you crave this kind of validation, and develop strategies to manage those feelings in less extreme ways.

You’re basically trying to have your cake and eat it too, saying that people who ascribe to an ideology don’t actually believe in the ideology they adopted. Unfortunately that’s not how it works in the real world. If they don’t ascribe to the ideology, then they don’t ascribe to the ideology, and that’s fine, but that doesn’t change the nature of the ideology itself. People don’t choose hateful and violent ideologies because they disagree with the ideology’s tenets. If they do disagree, they obviously don’t rush out and adopt the ideology they disagree with. You can’t have it both ways.

If they don’t agree with the ideology’s hateful and violent tenets, they don’t have to adopt the ideology, but it’s disingenuous to suggest that people who do voluntarily choose adopt those ideologies don’t actually believe in them. Nobody chooses to adopt an ideology they disagree with.
